Job description
Acute Care Case Manager
This is a 13-week contract position.
Day shift, 5 days a week from 08:00 AM to 04:30 PM, with weekend coverage required every other weekend.
The Acute Care Case Manager plays a crucial role in the discharge planning process and must effectively manage a caseload of up to 25 patients, ensuring timely and coordinated patient care. The position involves collaboration with multi-disciplinary teams and coordination of care across various services to facilitate smooth transitions of care for patients.
Conduct biopsychosocial assessments and develop individualized treatment plans.
Facilitate discharge planning and contingency planning.
Coordinate services to home care and equipment following discharge.
Document care delivery, plans, and patient interactions in the electronic charting system.
Educate patients and caregivers about care processes and resources available.
Participate in family meetings and collaborate with external resources as needed.
2 years of acute hospital case management experience required.
Experience with the Epic charting system.
Knowledge in handling involuntary holds and psychiatric assessment protocols.
CA state license preferred; pending license accepted.
