Case Manager 035 - New Brunswick NJ
Salary: $45,000 per year + $2,500 sign on bonus

Key Responsibilities:
Provide case management services to individuals residing in the Ozanam Inn Shelter.
Screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching.
Transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies and utilization of HMIS (WellSky) case management software and MyEvolv.
Requires on-call coverage and evening hours.
Bi-lingual (Spanish) encouraged to apply.
Ability to work independently.
Requirements: Bachelor's degree (B.A), preferably in Social Work or a similar Human Service field is preferred.
Experience with conflict resolution and mediation.
At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
Must have valid driver's license with acceptable driving history.
