nPOSITION SUMMARY:
nThe person in this position will provide case management services to HOPWA eligible individuals and their families to support their transition to self-sufficiency. Responsibilities include arranging, coordinating, monitoring, and delivering services that address clients’ housing needs and promote housing stability. Essential Job Functions include, but are not limited to, those listed below. Additional duties may be assigned as needed.
nESSENTIAL JOB FUNCTIONS:
nnnOversight of day-to-day operations of the program;
n
nnBuild and maintain relationships with property owners/managers to secure units for leasing;
n
nnMeet with each client at least once weekly to provide support and assistance towards achieving each Individualized Plan;
n
nnIntake and assessment of incoming participants to determine eligibility and appropriate services;
n
nnMake referrals and appointments with appropriate agencies, services and/or case managers;
n
nnMaintain client files according to company policy and contract requirements;
n
nnKnowledge of the Clinical Data System and HMIS (Homeless Management Information System);
n
nnProvide daily, weekly, and monthly reports as requested;
n
nnAssist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested;
n
nnMaintain relationships with referring and supporting agencies and represent the agency at various community meetings and outreach events as requested;
n
nnCoordination of services across multiple agencies;
n
nnAttend scheduled meetings and presentations with referring agencies;
n
nnPerform any other duties as assigned.

n nQualifications
nESSENTIAL QUALIFICATIONS:

nExperience and Competencies:
nnnExcellent communication and public speaking skills .

nnAbility to obtain and maintain clearance from LEIE, MSIL, and EPLS.

nnValid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required.

nnAbility to obtain and maintain a successful background clearance.

nnDuring your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
n
nnCPR Certification, First Aid Certification, and an annual Tuberculosis test.
n
n nEDUCATION and/or EXPERIENCE:
nnnDemonstrate knowledge of housing services including one (1) year experience working in similar programs; working with the homeless population, substance use disorder and/or mental health care systems is required.
n
nnDemonstrate knowledge of target population including one (1) year experience serving people living with HIV is required.
n
nnA minimum of one (1) year experience implementing trauma-informed approaches is required.
n
nnA minimum of one (1) year experience with data collection, data analysis, and confidentiality is required.
n
nnGraduation from an accredited two-year college with a degree in Addiction Counseling, Counseling, Social Work, Psychology, or a related field is preferred.
n
nnHigh School Diploma or equivalent is required.
n
n nLICENSURE/CERTIFICATION:
nnNot applicable for this position.
n nWORKING CONDITIONS:
nnnWork is primarily performed in an office setting; and
n
nnSome outdoor activities may be required.
