nPOSITION SUMMARY:
nThe Case Manager is responsible for providing intensive, person-centered case management services to veterans and service members who are identified as at risk for suicide. This position assesses participant needs, reduces barriers to care, coordinates services, and supports stabilization, recovery, and long-term wellness. The Case Manager works closely with program, intervention, and peer support staff to ensure participants receive timely access to mental health care, substance use treatment, housing, employment support, benefits, and other wraparound services. Essential functions include those listed below.
nESSENTIAL JOB FUNCTIONS:
nnnConduct comprehensive, person-centered assessments to identify participant strengths, needs, risks, barriers, and service priorities.
n
nnDevelop, implement, monitor, and update individualized service plans in collaboration with participants and the interdisciplinary program team.
n
nnProvide ongoing case management, care coordination, advocacy, follow-up, and resource navigation for veterans and service members at risk for suicide.
n
nnConnect participants to behavioral health, substance use treatment, medical care, housing, employment, benefits, transportation, legal, and other community-based resources as appropriate.
n
nnMaintain regular participant contact and promptly address barriers that may interfere with engagement, safety, stabilization, or continuity of care.
n
nnCoordinate with peer support staff, health care providers, community partners, veteran-serving organizations, and other referral sources to promote continuity of services.
n
nnSupport suicide prevention and risk-reduction activities within the scope of the psooition and follow established program protocols when safety concerns are identified.
n
nnProvide trauma-informed, culturally responsive, strengths-based, and participant-centered services while maintaining appropriate professional boundaries.
n
nnDocument assessments, service plans, contact, referrals, progress, outcomes, and other required information accurately and within established timeframes.
n
nnParticipate in case conferences, staff meetings, training, quality improvement activities, and grant-related data collection and reporting as assigned
n
nnMaintain confidentiality and comply with organizational policies, funder requirements, and applicable laws and regulations.
n
nnConduct community-based outreach and participant visits as needed.
n
nnMust Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and
n
nnPerform all other duties as assigned.

n nQualifications
nESSENTIAL QUALIFICATIONS:

nExperience and Competencies:
nnnStrong interpersonal, engagement, advocacy, and relationship-building skills.

nnAbility to organize and prioritize a caseload, coordinate multiple services, and complete documentation timely.

nnAbility to communicate effectively with participants, families, providers, community partners, and multidisciplinary teams.
n
nnSound judgment, problem-solving ability, and the ability to respond appropriately to urgent participant needs.
n
nnBasic computer proficiency and ability to use electronic records, databases, email, and standard office applications.
n
nnExcellent communication skills;
n
nnAbility to exercise good judgment and discretion;
n
nnAbility to work well in a team environment;
n
nnMilitary/Veteran status required.
n
nnAdherence to the highest standard of ethical conduct, especially to standards governing confidentiality;

nnProfessional appearance and demeanor;

nnValid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required; and
n
nnAbility to obtain and maintain a criminal history background check.

nnDuring your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
n
n nEDUCATION and/or EXPERIENCE:
nnnMinimum of two (2) years of experience in case management, behavioral health, veteran services, or a closely related area.
n
nnExperience working with high-risk populations, including individuals experiencing mental health and/or substance use disorders.
n
nnKnowledge of suicide prevention principles, trauma-informed care, community resources, and challenges affecting veterans and service members.
n
nnMinimum of two (2) years of experience in case management, behavioral health, veteran services, or a closely related area.
n
n nLICENSURE/CERTIFICATION:
nnNot applicable for this position.
n nWORKING CONDITIONS:
nnnWork is primarily performed in an office setting.
n
nnRegular travel throughout the assigned service area is required to support outreach, community partnerships, participant services, meetings, and program activities.
n
nnTravel between WestCare facilities, VA facilities, community partner locations, and outreach sites may be required.
n
nnPosition requires regular interaction with Veterans and individuals who may be experiencing behavioral health concerns, emotional distress, significant psychosocial stressors, or suicide risk.
n
nnSome evening or weekend work may be required to support outreach events, community activities, or program needs.
