nPOSITION SUMMARY:
nThe Case Manager provides comprehensive, person-centered case management and care coordination services to individuals served by the Wisconsin Mobile Mental Health Unit. The position conducts psychosocial and needs assessments, assists with crisis stabilization, develops individualized service plans, coordinates referrals, provides brief supportive interventions, facilitates psychoeducational activities, and connects individuals with behavioral health, medical, housing, financial, and social supports. The Case Manager works in community-based settings and is responsible for safely operating the Mobile Mental Health Unit, transporting clients as appropriate, conducting outreach, and supporting engagement in ongoing treatment and long-term stability. Essential functions include those listed below.
nESSENTIAL JOB FUNCTIONS:
nnnEngage individuals experiencing behavioral health concerns or crises using a respectful, trauma-informed, person-centered approach.
n
nnConduct psychosocial, functional, and service-needs assessments and identify immediate barriers to safety, stabilization, treatment, and recovery.
n
nnAssist the clinical team with crisis stabilization activities and implementation of individualized safety, service, and follow-up plans.
n
nnDevelop, implement, monitor, and update individualized service plans in collaboration with the individual, treatment team, family or natural supports when appropriate, and community providers.
n
nnProvide case management, care coordination, brief supportive interventions, education, advocacy, and problem-solving assistance within the scope of the position.
n
nnCoordinate referrals and warm handoffs to behavioral health, substance use, medical, housing, benefits, employment, transportation, financial, and other community-based services.
n
nnFollow up on referrals and services to support continuity of care, reduce barriers to treatment, and promote successful engagement after a crisis encounter.
n
nnConduct community outreach and engagement activities with individuals who may have difficulty accessing or remaining connected to services.
n
nnFacilitate or assist with psychoeducational, wellness, recovery, and skill-building groups as assigned.
n
nnMaintain timely, accurate, objective, and complete documentation of assessments, contacts, referrals, services, follow-up activities, and outcomes in accordance with program and organizational requirements.
n
nnCommunicate significant changes in an individual's condition, safety, functioning, or service needs to the Clinical Director and appropriate members of the treatment team.
n
nnParticipate in interdisciplinary case consultation, care coordination meetings, staff meetings, supervision, training, and quality-improvement activities.
n
nnMaintain current knowledge of community resources and develop collaborative working relationships with hospitals, behavioral health providers, social service agencies, shelters, law enforcement or first responders when applicable, and other referral partners.
n
nnSafely operate the Mobile Mental Health Unit and other authorized program vehicles; complete required vehicle checks and follow organizational transportation and safety procedures.
n
nnTransport clients to services, appointments, other approved destinations when appropriate and authorized by program procedure.
n
nnProtect client confidentiality and handle protected information in accordance with organizational policy and applicable privacy requirements.
n
nnFollow crisis-response, safety, incident-reporting, infection-control, and emergency procedures and immediately report safety concerns through the appropriate chain of command.
n

nnPerform all other duties as assigned.

nnCPR Certification, First Aid Certification, and an annual Tuberculosis test.

nnValid Commercial Driver's License (CDL) required, with an acceptable driving record and the ability to obtain and maintain eligibility for WestCare’s Vehicle Insurance.
n
nnDuring your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
n
n nEDUCATION and/or EXPERIENCE:
nnnMinimum one (1) year of experience providing case management, behavioral health, crisis intervention, community outreach, or human services preferred.
n
nnExperience working with individuals experiencing mental illness, substance use disorders, co-occurring disorders, housing instability, or other complex service needs preferred.
n
nnKnowledge of community behavioral health systems, social service resources, care coordination practices, and barriers affecting access to services.
n
nnBachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology, or a related field is strongly preferred.
n
n nLICENSURE/CERTIFICATION:

n nWORKING CONDITIONS:
nnnWork is primarily performed in an office and community-based environment.
n
nnRegular travel throughout the assigned Wisconsin service area is required to support outreach, community partnerships, participant services, meetings, and program activities.
n
nnRegular local travel throughout the program service area is required

nnSchedule may vary based on program operations and may include evenings, weekends, holidays, or other nontraditional hours when assigned.
