Case Manager Opportunity At Unity House Of Troy
Unity House of Troy is seeking a compassionate and dedicated Case Manager to join our Community Residence team located on 8th Street. If you enjoy supporting individuals on a personal level and helping them navigate day-to-day routines, this may be the perfect opportunity for you.
Key Responsibilities
Foster a structured, supportive, and inclusive environment for residents.
Build positive, professional relationships with consumers.
Assist with daily living skills, including cooking, budgeting, and personal care.
Support residents with medication management.
Ensure the facility remains clean, safe, and well-functioning.
Provide supportive counseling and offer encouragement and positive feedback.
Transport clients and complete program-related errands as needed.
Promote a culture of belonging and respect.
Qualifications
High School Diploma or GED required.
Minimum 1 year of experience working with adults living with mental illness.
Valid NYS Driver License and reliable transportation.
Strong verbal and written communication skills.
Basic computer proficiency.
Ability to work independently and collaboratively as part of a team.
