nElderwood Assisted Living at Waverly is in growth mode and currently looking for a dedicated and compassionate Case Manager to join our team!

nCase Manager's assist with ensuring the health and well-being of our residents by being responsible for the admission of residents; providing case management services for residents; and serving as the designated Discharge Planner for the facility.

nResponsibilities

nCase Manager

nnnConsistently demonstrates Elderwood iCare Values.

n

nnEvaluate initially and periodically, at least once every twelve (12) months, the needs of a resident and the ability of the program to meet those needs.

n

nnOrient new residents to the daily routine, including introductions to staff and facility programs.

n

nnIdentify adjustment problems and assist the resident to resolve such problems if needed.

n

nnAssist each resident to maintain family and community ties and to develop new ones.

n

nnEncourage resident participation in facility and community activities.

n

nnIdentify needs and arrange for services and entitlements from public and private sources for income, health, mental health and social services (if applicable).

n

nnAssist residents in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual’s health and mental health needs.

n

nnCoordinate the work of other case management and service providers within the facility.

n

nnAssist residents in need of alternative living arrangements; to make and execute sound discharge or transfer plans.

n

nnSchedule appointments and arrange for transportation of residents to other facilities, as necessary.

n

nnAssist residents as needed in shopping for personal items such as toiletries, medical supplies, stationery, books and clothing.

n

nnEstablish and maintain a system of record keeping that documents the case management needs of residents and activities undertaken to meet those needs.

n

nnCooperate with outside service providers in executing care plans for residents (if applicable).

n

nnResponsible for the completion of appropriate assessments of resident prior to admission; responsible for the on-going completion of required documentation.

n

nnAssist in arranging for transportation, as necessary, to ensure that residents are able to attend required/desired services provided outside the facility.

n

nnParticipate in the development of a plan of care for residents in cooperation with the Resident Care Manager (if applicable).

n

nnGive tours and presentations to potential residents, and families when necessary.

n

nnMeet regularly or as needed with Sales Manager to ensure inquiry/admission needs are met.

n

nnReceive completed admission applications and record names of new residents in the Admission/Discharge Log; coordinate obtaining of records or filing of records upon admission or discharge.

n

nnReview and complete Admission Agreement and documentation with resident and/or responsible party in cooperation with the Residential Sales Department.

n

nnExplain the Bill of Rights for Residents and Resident Abuse Reporting at General Orientation and in-service training, as required.

n

nnCoordinate facility cooperation with the Ombudsman.

n

nnServe as coordinator of Lost/Found possessions of residents.

n

nnServe as coordinator to whom facility staff report a violation of residents’ rights.

n

nnServe as coordinator of room/apartment reservations for residents, when necessary.

n

nnFollow-up on complaints and grievances of residents, responsible party or family members in cooperation with the Administrator.

n

nnMeet on a regular basis with the Administrator to discuss department operations; submit a monthly report to the Administrator.

n

nnMaintain a record of the number of overnight visits of Medicaid residents (if applicable).

n

nnComplete quality assessment audits and ensure follow-up of results that do not meet facility standards; implements changes as needed.

n

nnReview annually and update, if necessary, the department policy and procedure manual.

n

nnEnsure that departmental budget goals are met and costs are controlled as required.

n

nnAttend Morning Report, Admission Committee meetings, Marketing Committee, Quality Assessment and Review Committee, Staff meetings and required or preferred in-service training.

n

nnArrives to work on time, regularly, and works as scheduled; responsible for own transportation.

n

nnUtilizes electronic timekeeping system.

n

nnObserves and practices confidentiality of resident information and privacy when performing job duties.

n

nnRecognizes and follows the dress code of the facility including wearing name tag at all times.

n

nnPerforms assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.

n

nnAttends facility meetings, staff meetings, and required or preferred in-services.

n

nnGenerates and develops ideas which improve the quality of care for residents or increase position productivity and satisfaction.

n

nnPerforms all position duties according to safety rules and practices required for this position, and as required to ensure the general safety of staff, residents and visitors of this facility.

n

nnDemonstrates knowledge and understanding of all policies and procedures and ability to reference them.

n

nnExhibits the highest level of customer service towards residents, staff and visitors.

n

nnConsistently demonstrates the ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.

n

n nQualifications

nCase Manager

nnnBachelor’s degree in social work or related field and one year full-time experience in provision of services to the older adult population; or an associate’s degree in social work or related field and three years full-time experience in provision of services to older adult population.

n

nnValid New York Drivers License and clean driving record required.

n

nnKnowledge in Microsoft Office programs preferred.

n

nnCase work experience required; case work with the elderly or work in a supervisory capacity requiring initiative and responsiveness to the needs of others preferred.

n

nnAbility to meet the public easily, handle problems diplomatically, and cooperate with other staff members necessary.

n

nnEvidence of professional growth, continuing education and community involvement considered.

n

nnAbility/Willingness to work closely with the Sales Manager.

n

nnThis position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
