Key Responsibilities
Collaborate effectively with the RN Case Manager and interdisciplinary teams.
Maintain detailed documentation of patient care and progress.
Communicate clearly with patients, families, and healthcare professionals.
Adapt promptly to shifting priorities and various clinical tasks.
Build trust and rapport with patients and their families.
Qualifications
Required Skills: Strong documentation skills.
Excellent communication skills.
Able to manage multiple tasks effectively.
Proficient in building patient and family trust.
