Overview

nStarting Salary: $19.50/hour

nSchedule:

nThe Day Services Case Manager supports individuals in a day program setting by helping design and implement engaging, person-centered activities that promote independence, skill development, and community involvement.

nYou will work as part of an interdisciplinary team to develop activity schedules, provide direct skills training, and support individuals in achieving their personal goals. This role also includes documentation, progress notes, and collaboration with families and team members to ensure high-quality, coordinated supports.

nMinimum Education Required

nHigh School Diploma/GED

nResponsibilities

nnnAssist in developing curriculum and structured programming to promote independence and growth

n

nnDesign and implement activity schedules and therapeutic/recreational programming

n

nnProvide direct skills training and individualized support

n

nnDevelop creative, innovative programming to enhance engagement and empowerment

n

nnCollect and maintain daily data, progress notes, and semi-annual reports

n

nnProvide direct care and support with activities of daily living as needed

n

nnMaintain active communication with families, funders, consultants, and team members

n

nnParticipate in team meetings, supervision, and required trainings

n

nnMaintain certifications including First Aid, CPR, Safety Care, and HR/DPPC

n

nnEnsure all services align with Advocates’ Human Rights Policy and organizational values

n

n nQualifications

nnnAssociate’s degree in a related field and 1 year of experience or high school diploma or equivalent and 2+ years of relevant human services experience in lieu of a degree

n

nnValid driver’s license, reliable vehicle, and willingness to transport individuals

n

nnStrong computer skills required

n

nnAbility to communicate effectively in English

n

nnStrong written communication and documentation skills

n

nnAbility to multitask in a fast-paced, changing environment

n

nnAbility to work independently and as part of a team

n

nnCommitment to person-centered, respectful support practices

n

nnExperience working with diverse populations preferred

n

n nPhysical Demands:

nnnFull upper extremity range of motion

n

nnFull lower extremity range of motion

n

nnMust be able to lift furniture and/or objects/persons weighing over 50 lbs.

n

n nPhysical Activity:

nnnContinuous: Communication

n

nnFrequent: Standing, sitting, walking, lifting

n

nnOccasional: Reaching with hands and arms, stooping and crouching

n
