Job Title
Catholic Charities is working closely with the City of San Jose Mayor's Gang Prevention Task Force to provide Case Management Services, Outreach Services, Weekly Support Groups and Pro-Social activities to youth according to Hot Spot's identified on the City of San Jose BEST Contract.
The case manager implements outreach and support to empower youth & their families in making healthy choices.
Essential Duties and Responsibilities
Administrative Duties:
Researches and has a working knowledge of community resources
Provides needs assessment to address through linkage and case management services
Assists and links families to needed internal and external resources to promote overall well-being and improved mental health
Assist families with service provider process navigation to ensure service obtainment
Ensure that all required forms, consents, family plans, legal documentation, etc. are properly completed and kept up to date
Transfer family and service documentation to electronic data systems as necessary
Manage and maintain a caseload of youth and families
Prepare weekly and monthly clientele reports on Excel spreadsheets
Commit to other administrative duties such as creating a weekly schedule, maintaining reimbursement reports, online timesheets, and clientele weekly and monthly reports
Meet with direct supervisor on a weekly basis to discuss clients, program adjustments, required support, etc.
Programmatic Duties:
Provide Pro Social Activities on Tuesday & Thursday evenings
Provide transition services to integrate the youth back into school and to help the youth succeed once there
Establish and maintain good working relationships with school administration, community partners, SJPD and Juvenile Probation Department
Implement program evaluation procedures such as pre and post surveys, assessments and ISP's
Represent Children, Youth and Family Development at community events
Assist team with coordination of services
Provide after hour support over the phone for clients in need
Advocate for client in court, school, probation, and "referring-out" circumstances
Provide a weekly 1-1 (School or Home Visit with each youth as needed)
Qualifications
Education: BA preferred and/or extensive equivalent work experience/knowledge (Equivalent education and experience that demonstrates the ability to perform the job may be considered.)
Bilingual is a plus.
Required Knowledge, Skills and Abilities, Experience
Desire to work autonomously and in a team-setting.
Implements the attitude to balance administrative duties as well as working with families and the community at-large.
Case Management/Counseling experience in a community based setting.
Effective verbal and written communication skills.
Understanding and sensitivity of economic, cultural issues in diverse communities.
Ability to work independently with minimal supervision.
Ability to meet deadlines.
Demonstrated computer literacy (MS Word, Excel, and Internet usage).
Fingerprinting and background check as required by the DOJ and DMV clearance.
TB test required
Physical, Emotional and Intellectual Requirements
Extensive knowledge of how to work with at-risk and gang impacted youth.
Knowledge of asset-based youth development strategies.
Ability to collaborate with CBO's & agencies working with at-risk gang related or gang impacted youth.
Experience working with diverse populations.
Experience working with youth in the juvenile probation system.
Fast paced, diverse and dynamic environment.
The service is sometimes physical, requiring setting up for events which may require transporting or lifting cumbersome materials, e.g., easels, flip charts, refreshments, brochures, chairs, tables, etc.
Must be able to report to service site on a regular and reliable basis.
Able to deal with stressful situations.
Must be able to work a flexible schedule, which includes day, evening, weekend, and holiday hours, as appropriate, to coordinate and provide services and activities for families during non-traditional hours.
Able to travel between sites and to attend offsite events throughout the city, as appropriate.
