nPosition: Case Manager
nThe Case Manager is responsible for the delivery of care coordination services to a population with history of substance abuse. The Case Manager serves as a member of the interdisciplinary treatment team.
nJob Scope:
nnnCase management and crisis intervention to assist in the coordination of treatment services in line with workforce initiatives and promote access to transitional support services and clients dealing with sanctions, barriers to employment and self-sufficiency.

nnFacilitation of support group such as life skills
n
nnAdvocates on behalf of clients to obtain needed services.
n
nnInterfacing with appropriate agencies and systems (DSS, probation, courts, etc.) on behalf of the client and assisting clients in meeting all mandates.
n
nnOngoing case management related to housing, transportation, childcare, food and other related needs.

nnConducts toxicology tests, as needed.
n
nnMaintain communication and treatment planning and direct service providers and Suffolk county vocational service providers.
n
nnSubmit weekly schedules, ensure prior approval for off-site activities, monitor and enhance effective utilization of time.
n
nnAttend and participate in staff/community meeting where appropriate.
n
nnProvide crisis intervention and group work services and back up coverage, as needed.
n
nnMaintain computer skills that are appropriate to the level needed for optimal job performance.
n
nnExercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
n
nnInsure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan and personnel policy and procedure.
n
nnEach role at Outreach has identified production and outcome goals. These goals may be stated as a number or as an outcome within an identified time frame. These goals are intended to provide each job holder the knowledge they need about expected performance. Accomplishments against these goals will be a discussion point in the annual evaluation.

nAssistant Program Director in collaboration with Program Director
nQualifications
nnnCASAC-T; CASAC, or higher preferred.
n
nn1 year of experience working with clients with substance abuse disorders.
n
nn1 year of experience working with adolescents with co-occurring and mental health problems.
n
nn1 year of experience working in an OASAS funded facility a plus
n
nnCommitment to providing quality services and outcome driven performance measures.
n
nnExcellent organizational management skills with the ability to coach staff, manage, and develop high-performance clinicians, set and achieve program objectives.
n
nnStrong networking and public relations skills with the ability to engage a wide range of stakeholders, populations, and cultures.
n
nnAction-oriented, adaptable, and innovative approach to program planning.
n
nnAbility to work effectively in collaboration with diverse groups of people
