As a Case Manager, you will work collaboratively with clients, community partners, and internal teams to connect individuals with essential resources, develop personalized support plans, and advocate for the well-being of those we serve. This position is a full-time, non-exempt role, primarily working Monday through Friday, with occasional Saturdays as needed.

Participates in program monitoring, quality assurance, audits, and case conferencing as required; implements corrective actions and process improvements in coordination with leadership.

Knows, follows, and enforces Association policies, confidentiality requirements, and all safety/emergency procedures, including those related to mandated reporting and the prevention of child abuse.

Participates in required meetings and trainings; supports program initiatives, team operations, and other duties as assigned.

Requirements Bachelor's degree required; Master's degree in a related field preferred. Significant relevant experience may be considered in lieu of degree requirements in some cases.

Prior case management experience and demonstrated ability to support individuals experiencing homelessness or housing instability; experience working with co-occurring needs (e.g., mental health/substance use) preferred.

Knowledge of person-centered planning, motivational interviewing, and trauma-informed approaches preferred.

Strong documentation skills, computer proficiency, and ability to manage multiple priorities while meeting deadlines and compliance requirements.
