Job description
Job Overview The Care Manager serves as the primary point of contact between members and the health plan. This role is responsible for coordinating care, reviewing person-centered service plans, and ensuring members receive the appropriate covered and non-covered services.
Key Responsibilities
Conduct monthly member outreach and follow-up
Address and respond to identified member needs
Coordinate care with members, families, and vendor agencies
Maintain timely and accurate documentation
Participate in weekly team meetings
Perform additional duties as assigned
RN, LMSW, MSW - bilingual a plus - Chinese, Mandarin, Urdu, Vietnamese, Spanish
