nUsing a harm reduction approach in a “stages of change” model, the Case Manager will work with clients to assess barriers and establish goals with a focus on linkage to treatment, health education and achieving self-management. Frequent contact and follow-up with clients will support them in staying in care and decreasing health-related crises. The Case Manager will focus on all seven counties of Western New York, including Erie, Niagara, Orleans, Chautauqua, Wyoming, Cattaraugus, and Allegany county. The Case Manager will provide health education to PLWHA throughout the region, including individual and group education on topics related to treatment options, viral suppression and skills needed for self-care. The Case Manager will, using a structured curriculum in group and individual activities, engage participants in programming that will support behavior change.
nAs part of the essential functions of this role, the Case Manager:
nnnConducts initial patient assessment and develops treatment/service plan
n
nnConducts health education screening and develops health education plans
n
nnEngages with and builds rapport with patients, establishing weekly contact to assist patients with action steps toward reaching their goals
n
nnProvides case management for clients by arranging appropriate and necessary service linkages (e.g. medical, legal, housing, social, financial); represents client when advocacy is necessary
n
nnProvides support and linkages to clients based on case management psychosocial assessment, addressing counseling needs and making appropriate behavioral health referrals
n
nnAdvocates for services that support self-sufficiency and self-management
n
nnAttends/arranges case conferences with other service providers
n
nnProvides health education to clients regarding HIV transmission and harm reduction
n
nnAssists in the planning and implementation of health education groups.

nnCollaborates with team members to make sure patient needs are being met
n
nnOther duties as requested
n
n nQualified candidate will have a Bachelor's degree with two (2) years of field experience in HIV/AIDS, behavioral health, substance use or other chronic illness; h ave and maintain a valid NYS driver's license (verified through NYS LENS system) and insured, dependable car to use for patient/program activities, including transporting patients when necessary; Sensitivity toward cultural diversity, people living with HIV/AIDS, and lifestyle and addiction issues essential.
