Minimum Qualifications:
Bachelor's degree in Social Work, Psychology, or a related field.
Must be fluent in Spanish
Experience in case management or social services, particularly in a health care setting.
Strong understanding of mental health issues and crisis intervention techniques.
Preferred Qualifications: Master's degree in Social Work or a related field.
Certification in case management or a related specialty.
Experience working with child/family welfare programs or in program development.
Responsibilities: Conduct thorough assessments of clients' needs and circumstances to develop personalized care plans.
Coordinate and monitor services provided to clients, ensuring they receive appropriate support and resources.
Facilitate communication between clients, families, and service providers to promote collaboration and understanding.

Evaluate and adjust care plans regularly based on client progress and changing needs.
