Job description
Empower HER Foundation is seeking a dedicated Case Manager to provide home-based family services focused on child safety, family stabilization, and prevention of out-of-home placement. This role involves direct engagement with families, case planning, and coordination of services in alignment with DCFS standards.
Key Responsibilities
Conduct home visits and family assessments
Develop and implement individualized service plans
Monitor child safety and family progress
Coordinate referrals to community resources
Maintain accurate, timely case documentation
Participate in weekly supervision and case reviews
Collaborate with DCFS and community partners
Qualifications
Bachelor’s degree in Social Work, Psychology, or related field (required)
1–3 years of experience in child welfare, case management, or human services preferred
Strong documentation and organizational skills
Ability to work independently in community settings
Reliable transportation required
Home visiting certification
