nJob Summary
nWith the general oversight of the CSS Program Manager, works from a trauma informed care perspective to perform a wide range of recovery-based services including but not limited to support, advocacy, linkage, and coordination of services to support engagement with behavioral health providers and collaboration between members of the treatment team. Individual will be located within the County Mental Health Counseling Center and will strive to support a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences .
nEssential Job Functions:
nnnCoordinate and participate in weekly treatment team meetings
n
nnProvide outreach calls and visits to high-risk clients
n
nnProvide outreach calls and visits to clients not following through on appointments
n
nnGather collateral information to ensure collaboration and communication amongst team members
n
nnMake calls to collaterals to discuss client concerns and updates
n
nnAssist in coordination of urgent medical assessments for those who are not lined with a primary care physician (PCP)
n
nnAssist in linkages to local Federally Qualified Health Centers (FQHC)
n
nnAssist in coordination of transportation
n
nnEnsure timely follow up with PCP
n
nnConduct home visits to evaluate home environment when there are concerns for safety/self-care
n
nnAssist with linkage to benefits to include but not limited to:
n
nnMedicaid/Medicare
n
nnSSI/SSD
n
nnPublic Assistance
n
nnLink individuals with community resources
n
nnEnsure “warm hand off” when making referrals to other services
n
nnUtilize service dollars (when available) to assist eligible individuals (SPMI) in gaining access to or supporting participation in services where there is no other means of support for that access or participation
n
nnMay work with both the adult and children & youth populations
n
nnMay assist in transporting clients to urgent services as needed.
n
nnCan effectively utilize Microsoft Office applications, navigate the internet, utilize program software and electronic health records for documentation.
n
nnAbility to effectively communicate with providers/care teams to ensure the client’s needs are met.
nEducation and Experience:
n
n nTypical qualifications are a high school diploma, or equivalent, with two (2) years of relevant experience or an associate’s degree in human services or related field. Valid NYS Driver’s License as driving is an essential function of the position.
nKnowledge:
nThorough knowledge of community services within service area; general knowledge and understanding of Mental Illnesses, Psychiatric Rehabilitation model and related issues; skills for independent living. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.

nUse of contemporary office equipment particularly a computer with word processing, database and report generating software; to communicate effectively with diverse individuals and to record notes as needed; to listen, understand and appreciate the experiences of clients; to establish rapport and meaningful professional relationships; to negotiate, instruct; to manage and resolve conflicts; to provide positive role-modeling; to inspire respect, confidence and trust in clients and co-workers; to respect and maintain appropriate confidentialities; to effectively encourage clients toward greater independence and self-sufficiency; to perceive and describe changes in behavior; to generate and maintain accurate records and reports as required; to seek, accept, and learn from supervisor and peer feedback; to organize time effectively; to plan and implement strategies consistent with client needs and overall organization goals, objectives, and standards; to meet deadlines regularly. Has a basic understanding of the importance of working from a trauma sensitive perspective.
