nWHAT WILL I BE DOING?

nCHI is looking for a full-time Case Manager for our Coram location. You will be responsible for ensuring that all program participants are provided with the services necessary to promote personal and economic independence, to have skills required to obtain, maintain, and retain permanent housing, and are living in well-maintained units. This is accomplished by performing the following functions:

nnnAdhere to and enforce program regulations, policies, and procedures. Ensure that all clients are receiving quality services that are following the program requirements.

nnAssist clients in developing an Independent Living Plan (ILP) by identifying needs and goals. Evaluate client progress on short- and long-term goals.

n

nnProvide information, referrals, counseling, crisis intervention, direct service, and advocacy services for all clients.

n

nnDevelop and maintain a resource network for referrals as related to the needs of the individuals being served in supervised programs. Collaborate with DSS and other service providers to assist clients in fulfilling ILP and program requirements.

nnAssist clients with direct services such as current and projected client budget, current resume as appropriate, assistance with ADL skills, etc.

n

nnResponsible for face-to-face meetings with clients.

n

nnInspect living space regularly as required by the program to assess clients ADL needs and program compliance to assist clients with maintaining housing.

n

nnLimit and document improper use of agency resources by clients such as unit damages, furniture, or appliances.

n

nnEvaluate, document, and inform the direct supervisor of physical plant needs of the housing unit and/or common areas.

n

nnComplete discharge plans with clients.

n

nnSubmit all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (housing logs, unit inspection sheets, progress notes, ILPs, authorization forms required by DSS, special projects, etc.). Maintain charts and files in an orderly fashion.

n nWHAT DO I NEED?

nnnEducation: Bachelor’s Degree in human services or related field, Master’s Degree in Social Work preferred. Extensive experience may be substituted for educational requirement.

n

nnExperience: Two years’ experience working in a related field such as homelessness or housing preferred.

n

nnCommunication: Excellent verbal and written communication skills. Proficiency in English required, bi-lingual (Spanish) is a plus.

n

nnComputer Skills: Basic competency in MS Windows, MS Office, and internet usage.

n

nnMath Skills: Ability to formulate simple financial budgets.

n

nnPhysical Performance: Ability to tour property, walk distances, and climb stairs.

n

nnReasoning Ability: Ability to prioritize, make appropriate decisions, and judgment calls.

n

nnOther Skills: Ability to relate to the underserved, homeless, and disabled populations. Ability to work independently and as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details.

nnnSalary: Bachelor’s degree: $24.04/hr ($50,000 annually), Master’s degree: $26.45/hr ($55,000 annually)

nnOpen shifts: (Sunday - Thursday, 10 a.m. - 6 p.m.)
