Job description
Job Tittle: RN Case Manager
Location: Riverside, CA 92505
Duration: 26 Weeks | 36 hr/week
Shift Time: 6:00 PM - 6:30 AM | Nights
Pay Local: $60 - $70/hr
Pay Travel: $2800 - $3200 Gross weekly
Position Overview
The RN Case Manager will provide professional nursing and case management services, coordinating patient care and collaborating with physicians, nurses, and other healthcare professionals to support quality outcomes and appropriate utilization of healthcare resources.
Key Responsibilities
Coordinate and manage patient care across the healthcare continuum.
Assess patient needs and develop appropriate care plans.
Collaborate with physicians and multidisciplinary teams.
Monitor patient progress and facilitate appropriate interventions.
Coordinate transitions of care, discharge planning, and follow-up services.
Ensure appropriate utilization of healthcare resources.
Maintain accurate and timely clinical documentation.
Advocate for patients and help address barriers to care.
Communicate effectively with patients, families, providers, and other care team members.
Requirements Active California RN License
BLS Certification
Case Management / RN Case Manager experience preferred
Must meet facility assessment and qualification requirements
