nAre you passionate about helping youth overcome challenges and build successful futures? Join our team and become a trusted advocate for youth living in a trauma-informed residential program. As a Case Manager, you'll help coordinate services, support educational success, and ensure youth receive the care they need while making a lasting impact every day.

nWhat You'll Do

nCoordinate case management services for youth in care.

nDevelop, implement and regularly update individualized service or treatment plans for youth.

nBegin discharge planning early in placement to ensure smooth transitions to home, independent living programs, etc.

nBuild positive relationships with youth, families, schools, therapists, and community partners.

nAssist with educational planning, life skills, and transition services.

nMaintain accurate documentation and ensure compliance with agency and DFPS requirements, including keeping youth files up-to-date.

nTransport youth to medical, dental, mental health, educational, court, and other scheduled appointments using agency vehicles.

nHelp schedule appointments and communicate with providers, staying abreast of follow-up appointments as well.

nParticipate in treatment team meetings and collaborate with direct care staff.

nSupport youth using trauma-informed practices and assist during crisis situations when needed.

nMaintain ethical boundaries, cultural competence, and a trauma-informed approach in all interactions.

nQualifications

nBachelor's degree in Social Work, Psychology, Education, Human Services, or a related field (or qualifying equivalent experience).

nA minimum of 1 to 3 years of related work experience required, but 3-5 years preferred.

nMinimum age of 23 years.

nValid Texas driver's license with a safe driving record.

nAbility to pass required background checks.

nExperience working with youth, foster care, residential treatment, or behavioral health preferred.
