Job description
PAM Health Rehabilitation Hospital of Dover is seeking a Case Manager to coordinate interdisciplinary care for inpatient populations. The role involves reviewing admission records to ensure proper resource use and determining appropriate level of care, assessing discharge needs, and coordinating discharge plans with the team.
