Job description
Children's Diagnostic & Treatment Center
Provides ongoing case management services for children and youth with special health care needs and their families within a medical home model of care. Coordinates appointments for specialists and schedules medical tests/exams/assessments for patients. Assists families with financial and social assistance and ensures participation in the continuous planning of patient care.
Education: Essential: Bachelor
Education Specialization: Essential: Social Work
Experience: Essential: One Year
Credentials: Essential: Basic Life Support
AHCA Level II Required
