Job Title
Case Manager / Healthcare Consultant 1

Western Suburbs of Chicago Illinois area
Position Description
Manage a caseload of LTSS/waiver members and conduct in-person member visits.
Roles Responsibilities
• Manage a caseload of LTSS/waiver members.
• Conduct comprehensive member assessments and determine healthcare needs.
• Develop, coordinate, implement, and monitor individualized care plans.
• Coordinate with case managers, providers, Medical Directors, and community resources to ensure quality care.
• Identify high-risk members and escalate complex cases when needed.
• Educate, coach, and motivate members to improve health outcomes and make informed healthcare decisions.
• Assist members in accessing benefits, services, and community resources.
• Maintain accurate case documentation and ensure compliance with company policies and regulatory guidelines.
• Conduct face-to-face member visits (50-75% travel required).
Skills
• Bachelor's degree (or Master's) in a human services field.
• Minimum 2 years of case management experience.
• Valid Illinois driver's license.
• Reliable transportation and willingness to travel.
• Experience with Microsoft Office and electronic health records (EHR).
• Managed care or discharge planning experience (preferred).
• Experience working with HIV/AIDS populations (preferred).
• Bilingual/multilingual candidates are a plus (preferred).
Education
• Bachelor's degree (or Master's) in a human services field.
