Expectations
As a Case Manager, you'll:
Own a caseload and guide clients through assessment, service planning, and referrals — always centering their voice and choice in the process
Connect people to what they need — substance abuse and mental health treatment, transportation, and coordinated care with partner providers
Show up in crisis moments — conducting risk assessments, coordinating crisis plans with psychiatric providers, and linking clients to hospitals and crisis teams when it matters most
Keep meticulous, compliant records that protect your clients and strengthen our programs
Drive shared efforts, meeting people where they are — sometimes literally, behind the wheel, getting them to the appointments that change their trajectory
What You Bring
A high school diploma or GED (bonus points for an Associate's or Bachelor's degree)
Willingness to learn and train but 1–2 years of case management or supportive services experience with homeless or underserved populations is a plus
A working knowledge of homelessness, mental health, and substance abuse issues
Sharp time-management skills
Communication skills built for conflict resolution
Integrity, patience, and a genuine commitment to the mission
Comfort with ambiguity and a fast-moving environment
A valid driver's license and a clean traffic record

Schedule: General business hours, Monday–Friday, with some evenings, weekends, as needed.
