Job description
PAM Health Specialty and Rehabilitation Hospitals of Miamisburg is seeking a Case Manager to oversee interdisciplinary coordination of care for a designated patient population. You will review inpatient admission records to ensure proper utilization of resources and determine admission level of care, while coordinating discharge planning with the care team.
