Job description
Title: Case Manager (JISH)
Unit: Scattered Site Housing - Justice Involved Supportive Housing
Reports to: Program Director
Status: Full Time, Regular, Non Exempt
Salary Range: $32.97 to $33.71 per hour (approximately $60,000 to $65,000 annually)
Location: Harlem, New York
Days/Hours: Monday to Friday, 9am to 5pm (some flexibility required)
This position is represented by OPEIU Local 153
Position Summary: The Case Manager will perform initial assessments, provide ongoing supportive services both in the field and in the office and maintain documentation requirements for justice involved clients with histories of mental illness, substance use and homelessness.
Essential Duties and Responsibilities:
Conduct intake needs assessments on clients eligible and accepted for housing services;
Create individualized service plans and Outcome Stars for each client. Document all client services, including successful and unsuccessful outreach attempts;
Conduct crisis intervention with clients, as needed;
Assess client's need for temporary rental subsidy and furniture/moving allowance;
Assist clients in obtaining all necessary documentation to facilitate benefits application process. Assist clients with completion of and submitting client's application to appropriate benefits provider agency (e.g., HRA, Social Security);
Act as a liaison to representatives of benefits administrations;
Act as liaison in conjunction with property management, between client and landlord, to mediate any landlord-tenant issues and
Provide ongoing counseling, supportive services needed to achieve goals stated in client service plan and Outcome Star.
Travel across the 5 boroughs on mass transportation as assigned and as needed to provide services to assigned participants.
Perform all other duties as assigned.
Qualifications
Education: Associate's Degree in Human Services or/and five years of experience working with special needs population; a High School Diploma and eight years of experience working with special population or a bachelor's degree in human services is preferred with at least 2 years of experience with the target population.
Work Experience: Candidate must have the ability to oversee a caseload of 15 clients. In addition, the candidate must be able to provide a collaborative assessment of needs, create and implement service plan, monitor progress and evaluate services provided to the individual client.
Computer Skills: Proficient in Microsoft Excel, Word, and Outlook with a working knowledge of PowerPoint.
Communication Skills: Excellent written and verbal communication skills.
General Work Behaviors/Skills: Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically with diverse internal and external individuals.
