n$28-$31 depending on experience

nOVERVIEW:

nThe Salvation Army is seeking applicants for a full-time case manager. This position is part of the Joseph McFee Center (JMC), a recovery focused transitional housing program for San Franciscans exiting residential treatment. The JMC program provides participants with the opportunity to build on progress made in early recovery and to work toward a healthy, independent and prosperous life. Under the supervision of the Program Director, the Case Manager’s main goal is to address the needs of JMC participants and cultivate a transformative community based on the program's principles of recovery, accountability, and service. The case manager is responsible for assessing client needs, developing goal plans, providing referral and resource coordination services, client advocacy, crisis intervention, monitoring and evaluating client progress, documenting, and reporting client interactions and services, collaborating with staff and partner organizations, empowering and educating clients and adhering to professional ethics while maintaining client confidentiality.

nDUTIES:

nnnPerform all duties according to policy and procedure and The Salvation Army’s philosophy of care.

n

nnManage an assigned case load of up to 25 program particiapnts.

n

nnAssessment and Intake: conduct interviews and administer assessments to understand the client’s needs, situations, and potential risks. Gather relevent information about the client’s background, family dynamics, living conditions or any other relevent factors.

n

nnBased on the assessment, collaborate with the client to create an individualized action plan that outlines specific goals and objectives as well as interventions that address the client’s unique needs.

n

nnEstablish and maintain relationships with partner organizations and referring agencies.

n

nnMonitor client progress and reassess their needs to ensure that the goal plan remains relevent and effective.

n

nnEducate and empower clients by equiping them with the knowledge and skills needed to improve their situation and become more self-sufficient.

n

nnRespond to and manage crisis situations cleints may encounter.

n

nnMaintain appropriate, professional, and ethical boundaries with staff and clients.

n

nnBe punctual, flexible, and reliable.

n

nnMaintain accurate client records on The Salvation Army Social Services Database.

n

nnAttend all required staff, Corps and Divisional meetings and trainings.

n

nnFacilitate small group instruction as indicated on the program schedule.

n

nnAttend community-based meetings as needed.

n

nnIdentify appropriate resources, programs, and services that can benefit the client and make referrals to these services.

n

nnCoordinate with various service providers to ensure the holistic needs of clients are met.

n

nnOther reasonably related duties assigned by the Program Director or Program Manager.

n

nnAssist in the planning, preparation, and execution of special functions such as graduations, holiday celebrations, group activities, or special classes.

n

n nQUALIFICATIONS:

nnnRequired:

n

nnHigh school diploma. (College course work in social services or psychology preferred).

n

nnStrong verbal and written communication skills.

n

nnThe ability to express empathy and foster a collaborative alliance with clients.

n

nnFamiliarity with San Francisco social services, government resources, community resources, healthcare services, mental health support and housing services.

n

nnExcellent organizational skills and ability to work with little direct supervision.

n

nnCulturally competent and able to work with a highly diverse population.

n

nnExcellent time management skills.

n

nnDesired:

n

nnValid CA Driver License and completion of organization’s driver safety training within introductory period.

n

nnExperience and ability working successfully with issues of substance abuse, mental illness, and other potential barriers to self-sufficiency.

n

nnBilingual in Spanish preferred.

n

nnKnowledge of co-occurring disorders and trauma informed environment.

n

nnStrong electronic communication skills. e.g., Outlook

n

nnProficient in Microsoft Word & Excel

n

n nPHYSICAL REQUIREMENTS:

nnnAbility to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead.

n

nnAbility to a lift 25 lbs.

n

nnAbility to operate telephone.

n

nnAbility to type 35 words per minute.

n
