nnJob Summary:

nProvides services to individuals with severe and persistent mental illnesses to safely maintain them in the least restrictive setting and reduce the need for psychiatric hospitalization while maintaining and enhancing the individual's optimal functioning level. Duties include clinical and crisis intervention, assessments, service planning, monitoring, linking to and coordinating services and advocacy.

nnEssential Duties and Responsibilities:

nnConduct intake and referral with new individuals served.

nConduct assessments of clients' current functioning and stability levels and monitor stability through therapeutic and crisis intervention and support, education, motivation, and direct skills training.

nComplete prescreens to determine appropriateness for hospitalizations/placements.

nInitiate and monitor Alternative Treatments Orders as needed/required.

nAct as an advocate for clients in the mental and public health service systems.

nAssist clients in securing basic needs, medical care, and clinical services.

nAssist in determining appropriate level of care and ensure individuals served are receiving appropriate level of care.

nDevelop service plans based on client needs and desires with client and service team.

nComplete transfers/discharges in a timely manner, when appropriate.

nAssure service plan continues to meet client needs and is being followed.

nAssure system paperwork is in place so services can be provided.

nMaintain completed documentation on all client and collateral contacts to make sure that all medical and financial needs are being met.

nAssist in crisis intervention by assessing the client's clinical needs and calming agitated individuals.

nMaintain collateral contacts and relationships with others important in the client's life.

nHalf day shift assigned as needed during regular business hours to cover client emergent/urgent needs.

nEnsure the rights of individuals served are protected in accordance with the Michigan Mental Health Code and agency policy.

nActively support and be knowledgeable about 1) culturally competent, recovery-based practices, 2) person-centered planning as a shared decision-making process with the individual who defines his or her own life goals and is assisted in developing a unique path toward those goals and 3) a trauma informed culture to aid clients in their recovery process.

nAssist with arranging transportation and linking to appointments through arranging public transit, Medicaid Transport and/or use of personal vehicle to provide transportation to individuals served, adhering to all traffic laws and rules of the road.

nCompletes other miscellaneous duties as assigned.

n nnRequired Skills/Abilities:

nTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. nnVerbal and written communication skills required.

nOrganizational and time management skills and ability to prioritize multiple tasks.

nKnowledge of Microsoft Office products required.

nAbility to exercise independent and sound judgment.

nCriminal records, driving and recipient rights screening required.

nValid Michigan driver's license, good driving record and the availability of licensed, insured vehicle for use on the job required.

n nnEducation and Experience:

nnMinimum one-year experience in the mental health field working with the identified target population preferred AND Minimum bachelor's degree in social work and a current LLBSW, LBSW, LLMSW, LMSW, LLPC, LPC or other licensure permitted by the State of Michigan preferred OR Bachelors' degree in human service field AND one year experience working with individuals with Severe and Persistent Mental Illness required.

nLived experiences with behavioral health are desired.

n nnPhysical Requirements:

nnPhysical ability to sit for prolonged periods of time. Walking, stooping, kneeling, crouching, or bending as appropriate.

nPhysical ability to work in front of and view a computer terminal for prolonged periods of time.

nMay occasionally require lifting up to 35 pounds.

nAbility to meet intervention certification requirements (MANDT).

n nnWork Conditions:

nnOffice environment

nLocal travel throughout the greater Kalamazoo area

nMay be exposed to bloodborne pathogens, infectious diseases, and parasites.
