nKEY ESSENTIAL FUNCTIONS
nClient Advocacy:
nnnConducts needs assessment and case management for assigned families
n
nnConducts intakes, home visits and apartment inspections.
n
nnHolds bi-weekly interviews with clients.
n
nnMaintains up to date files including client history and profile.
n
nnProvides information and referrals, arranges for provision of services, advocates and intervenes with government agencies, short-term counseling and assists in developing service plan with goal of restoring family independence.
n
nnCoordinates workshops, seminars and other activities for general client population.
n
nnPerforms other related duties as assigned.
n
nnMakes recommendations to improve level of services.
n
n nRelocation Services:
nnnMeet with Head of Household and begin filling out Section 8, NYCHA and/or Homeless Housing Application. Complete documentation in a timely manner.
n
nnMeet with Head of Household every two weeks to determine housing status and immediate needs.
n
nnContact landlord/broker to schedule appointment to see apartment.
n
nnFollow up with landlord/broker to assure that Section 8 package or other related documents was submitted.
n
nnSchedule Head of Household for Apartment Inspection with housing site.
n
nnFollow-up meeting with resident for Lease signing.
n
nnFollow-up meeting, after lease has been signed, then schedule resident for move out.
n
nnContact Public Assistance agency to request rent checks.
nCase Management and Record Keeping:
n
nnProvides case management and on site crisis intervention.
n
nnCompletes required service plans.
n
nnEducates participants with respect to good housekeeping and home-safety practices, budgeting and landlord/tenant relations, as necessary.
n
nnMaintains appropriate documentation of all tenant contacts.
n
nnDocuments monthly progress notes in the participants’ chart.
n
nnConducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive services.
n
nnReports any alleged child neglect or abuse to the ACS agency or the authorities.
n
n nREQUIREMENTS
nnnBA degree in relevant field (Masters degree preferred)
n
nnMinimum of 2 years of Case Management Experience
n
nnExperience working with homeless population(s)
n
nnHousing Knowledge
n
nnFamiliarity with community Resources
n
nnExcellent oral and written communication
