nNexus-Family Healing has an exciting opportunity for a Case Manager to join our team in our Kinship team in our St. Cloud office! This position will work specifically with relative/kinship caregivers.

nnPay - $52,000-$56,000

n nPosition Summary:
nOur Case Manager provides support and services to relatives caring for youth in St. Cloud, MN and within the surrounding area. When a relative cares for a child or sibling group removed from their homes, the advantages are tremendous for the children - but the challenges these relatives face can often be large.
nPrimary responsibilities:
nnnNetworking with community to reach relatives caring for youth either formally or informally
n
nnConduct targeted outreach to locate kinship families for youth
n
nnConnect relative/kinship families with needed services and information while providing direct support to the children and families as needed
n
nnFacilitates share & support groups for relative and kin caregivers.
n
nnEnsures regular ongoing communication with families
n
nnComplete Home studies and licensing for relative and kin foster care providers.
n
nnRecognize and value cultural differences in all aspects of work and service delivery
n
n nRequired Education and Licensure:
nnnRequires a Bachelor's degree in Social Work, Psychology or Human/Social Services related field.
n
nnValid MN Driver's license and auto insurance coverage required.
n
nnMust have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges
n
nnMust be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values
n
nnAbility to pass all necessary background screening requirements per DHS rules and regulations
n
