Case Manager LVN

nThe LPN/LVN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.
nMinimum Requirements:
nnnCurrent LPN/LVNlicensure in state practicing
n
nnAt least one year of Case Management experience preferred
n
nnComplies with all relevant professional standards ofpractice
n
nnCurrent CPR if applicable
n
nnTB questionnaire, PPD or chest x-ray if applicable
n
nnCurrent Health certificate (per contract or stateregulation)
n
nnMust meet all federal, state and local requirements
n
nnMust be at least 18 years of age
n
