Job description
Case Manager (RN)
We are seeking a dedicated and compassionate Case Manager (RN) to join our team. The ideal candidate will demonstrate exceptional patient care skills and a cooperative attitude while managing a variety of case complexities within our healthcare environment.
Responsibilities
Conduct comprehensive assessments of patients' needs and develop individualized care plans.
Coordinate with healthcare providers, patients, and families to ensure optimal care delivery.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on treatment options and community resources.
Facilitate communication between multidisciplinary teams to support patient outcomes.
Document all patient interactions and care plans in compliance with regulations.
Qualifications
Valid Registered Nurse (RN) license.
Minimum of one year of nursing experience, with case management experience preferred.
Strong communication, organizational, and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Knowledge of healthcare regulations and compliance standards.
Schedule
This position typically follows an 8 AM to 5 PM schedule, with flexibility as needed based on patient and organizational needs.
Additional Information
Orientation sessions are conducted weekly. All traveling nurses are expected to maintain current certifications as required for the position.
