The Case Manager - Member Services Representative plays a pivotal role in assessing members' needs, advocating for essential services, and ensuring progress through continuous support and follow-up. By working closely with members and community organizations, the Case Manager helps bridge the gap between individuals and the resources that empower them to grow and succeed. nResponsibilities:
nnnMember Needs Assessment: Conduct intake interviews to determine member needs, ensuring each individual receives personalized support tailored to their unique circumstances.
n
nnAdvocacy: Serve as an advocate for members by connecting them with essential services and resources, helping them navigate social, medical, and community-based programs.
n
nnProgress Tracking: Monitor and document each member’s progress, ensuring consistent follow-up to address evolving needs and challenges.
n
nnData Collection & Reporting: Collect, analyze, and summarize treatment results and compile statistics, providing accurate and timely reports for Wider Circle leadership to track program effectiveness and outcomes.
n
nnCollaboration: Work closely with community partners, healthcare providers, and social service organizations to ensure members receive comprehensive care, in line with Wider Circle’s commitment to LOVE and support for its community members.
n
nnMember Empowerment: Educate members on available resources, guiding them through processes that foster LEARNING and self-empowerment.
n
n nQualifications:
nnnExperience in case management, social work, or healthcare advocacy.
n
nnStrong communication and interpersonal skills with the ability to build trust and rapport with diverse community members.
n
nnExcellent organizational and time management abilities to handle multiple cases effectively.
n
nnAbility to collect, analyze, and report on data.
n
nnKnowledge of community-based services and social programs.
n
nnCommitment to the values of LOVE, LEARN, GROW, and a passion for supporting vulnerable populations.
n
n nRequirements
nnn3-5 years of experience working in the insurance industry
n
nnExcellent customer service skills
n
nnStrong written and verbal communication skills
n
nnAbility to explain complex policies in a straightforward, easy-to-understand manner.
n
nnDemonstrate ability to manage multiple priorities in a fast-paced environment.
n
nnConflict resolution skills and people management are required.
n
nnAbility to build trusting relationships
n
nnPreferred: Degree in Human Services, Social Work related field.
