Job description
NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. We believe that people can change, and we work to empower each individual in their own care.
We are currently seeking a Case Manager for our Parenting with Love and Limits Program. PLL Case Managers focus on supporting youth enrolled in the program (aged 10-17) and connect families to community resources. As a case manager for the Parenting with Love and Limits program, you will be trained in this evidence-based model and facilitate teen groups alongside a PLL Clinician.
Responsibilities Include:
Co-facilitates weekly PLL groups Coordinates and conducts home visits with clients and families as required by contract and program policy
Advocates for clients’ needs within the program as well as with external agencies and resources, as appropriate
Coordinates the transition of services as required
Maintains professional boundaries with clients, families, employees, stakeholders and others at all times
Completes documentation of treatment and services for clients in keeping with licensing / funding source and agency requirements
Participates in staff trainings and PLL Certification training, and maintains certifications as appropriate and required
Provides safe transportation for clients as necessary / required
Performs crisis prevention and interventions including de-escalation techniques and / or physical management as needed
Demonstrates computer literacy by utilizing Human Resource Information System (HRIS), Electronic Health Records, Email and other web based platforms
Completes other related duties as assigned Performs all duties in accordance with the agency’s policies and procedures

Salary/Hourly Rate: $23.00 - $24.00 per hour
