n School Based Case Manager. Bring Hope. Change Lives!

nThe NewPath Child & Family Solutions School Based Case Manager is responsible for assisting children and families in achieving stated goals and objectives and maximizing independence and daily functioning through interventions and advocacy through our partnership with New Miami School District.

nnLocation:

nThis position's home base is at our NewPath Butler Campus, 3570 Pleasant Ave, Hamilton, OH 45015 and will work directly within New Miami Schools in Butler County.

nClinical Counseling/Therapy Licensure Supervision Hours (Cost Covered)

nBoard Licensure Exam Cost Covered (Therapy, Counseling, and Nursing)

n nnYour Role:

nnConduct individual, family and group treatment interventions as indicated in the Individualized Service Plan.

nCoordinate the necessary evaluations and assessments in order to identify each client's needs, DLA-20, ISP, PHQ9, Ohio Scales etc.

nProvide transportation as appropriate and necessary to reach identified goals.

nMaintain and model appropriate interpersonal relationships.

nDeliver culturally informed services.

nMaintain professional objectivity and self-discipline.

nAdhere to agency risk assessments, policies, protocols, safety questionnaire, on-call procedure, safety planning, and corporate compliance.

nEnsure that required elements of the ECR/EHR are present, current and in accordance with agency certification and documentation standards.

n nnQualifications:

nnMinimum of High School diploma and 5 years' experience with SED children; OR a Bachelor's degree in relevant field and at least 6 months experience working with SED children.

nExcellent written and verbal skills.

nAbility to work flexible hours, including some evenings and weekends.

nInitiative and ability to work independently;

nA willingness and comfort level in providing community-based services to a diverse population of clients;

nExcellent problem-solving skills and the ability to remain calm and professional in crisis situation.

nMust be a minimum of 21 years of age, have a valid driver's license and proof of current auto insurance with no more than 5 points on your MVR.

nMust be able to pass a pre-employment background check & drug screen.
