nTitle: Case Manager

nProgram: Canopy Women’s Shelter

nSalary Range: $42,290-$46,290

nProgram Description:

nCanopy House provides 171 emergency shelter beds for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, occupational therapy, mental health services and housing placement assistance. Canopy House is a unique facility in that it is co-located with a supportive housing program that occupies the top 15 floors of the building and offers a street facing Article 28 clinic on the first floor dedicated to providing primary and behavioral health care services.

nPositions Description:

nUnder the supervision of the Director of Social Services and the Team Leader, the Case Manager conducts required assessments and provides case management and direct services to clients. In this role, the Case Manager supports clients in addressing identified needs and building the skills and stability necessary to regain control of their lives, achieve greater independence, and transition to permanent housing or other appropriate community-based housing options. The Case Manager also provides referrals and links clients to appropriate internal and external resources, assists with other cases as needed, and ensures all required protocols are followed. The Case Manager maintains client charts in accordance with program standards and ensures documentation remains complete, accurate, and audit-ready at all times, and performs other related duties as assigned.

nEssential Duties and Responsibilities:

nThe essential duties of the Case Manager include, but are not limited to, the following activities:

nnnConducts intake and assessments, makes necessary referrals for services, and completes/updates psycho-social assessments of clients

n

nnProvides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long-term objectives that can be achieved

n

nnInputs all client information and updates in a timely fashion into the CARES system as well as the client’s physical chart

n

nnWorks collaboratively with clients and other staff to identify and obtain financial entitlements, housing, and other concrete services

n

nnMaintains accurate and up-to-date written client documentation as required and in accordance with City/State oversight agency guidelines

n

nnGuides clients through individualized sessions on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols, and pursuing employment opportunities

n

nnMaintains liaison with representatives from community organizations

n

nnProvides escort services to clients’ appointments as needed

n

nnAttends and participates in the program meetings

n

nnPerforms other duties as assigned by the Team Leader, Director of Social Services, and/or Program Director

n

n nPhysical Activities:

nWhile performing the duties of this position, the Case Manager must be able to navigate the 6-story facility by foot, walking up and down the stairwells, to observe staff and clients in the event the elevator is out of service. The Case Manager must be able to stand and bend to review client files, put them away, and close them out. It is also part of the role of sitting at the computer workstation for extended periods to review the client data and update the information in the case management systems. The Case Manager must be able to lift or move material and equipment weighing up to 15 pounds.

nQualifications:

nnnBachelor’s degree from an accredited college or university is preferred.

n

nnAn Associate’s degree with one (1) year of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs populations.

n

nnA High School Diploma or GED equivalent with two (2) years of experience working with homeless, mentally ill and/or chemically addicted individuals or other special needs populations.

n

nnBilingual or multilingual a plus

n

n nPreferred Skills:

nnnTeam Player: Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible.

n

nnCommunication: Excellent oral, writing, and listening skills are a must.

n

nnOrganizational: The ability to work well within a highly pressurized, deadline environment, while meeting the short-term and long-term mandates of the program.

n

nnInterpersonal: An ability to interface with clients as well as all levels of staff.

n

nnComputer: Knowledge of case management software as well as proficiency in Microsoft Office Suite

n
