A nonprofit organization in Queens, New York is seeking a Case Manager to provide supportive services for formerly homeless individuals in permanent housing.
About the Opportunity:
Shift: Day shift
Schedule: Monday to Friday (with one late night)
Hours: 9am to 5pm (with one 11am to :pm)
Setting: On-site
Responsibilities: Provide direct case management services to a caseload of approximately 15 clients
Conduct home visits, assessments, psychosocial evaluations, and crisis interventions as needed
Assist clients with benefits, entitlements, identification, recertification paperwork, and other housing-related documentation
Connect clients to psychiatric, medical, substance use, job training, food, recreational, and community support services
Maintain accurate case records and coordinate ongoing support plans to help clients remain housed and progress toward recovery goals
Qualifications: At least 2 years of experience working with homeless adults and substance use issues
Bachelor's degree
Experience with harm reduction and motivational interviewing techniques
Excellent computer skills, including proficiency in Microsoft Word and Excel
Excellent writing and communication skills
Experience working with formerly homeless individuals, individuals in shelter settings, or clients with mental health needs
Desired Skills: Bilingual Spanish/English
Experience with NYC government benefits systems
Background in social services, social work, or mental health
