We're hiring a Case Manager to support individuals and families-especially those with SPMI -in achieving stability and independence.
Bronx, NY
$25 - $26/hour
Full-time (35 hrs/week)
Temp-to-Perm
What you'll do:
• Assess clients & develop service plans
• Connect to housing, healthcare & community resources
• Provide ongoing support, advocacy & documentation
What we're looking for:
Bachelor's
Experience with SPMI or vulnerable populations
Strong communication & organizational skills
