nWe are seeking a compassionate RN Case Manager who believes in our mission: helping patients live their best lives at home. In this role, you’ll coordinate care, ensure quality outcomes, and support patients and families with skilled nursing care.
nJob description:
nPerform patient assessments and develop individualized care plans.
nCoordinate services with physicians, caregivers, and the care team.
nProvide patient and caregiver education and ongoing support.
nSupervise LPNs and HHAs/CNAs to ensure high-quality care.
nReview, revise, and update plans of care as needed.
nMaintain accurate and timely clinical documentation.
nParticipate in case conferences and agency quality programs.
nWhat We’re Looking For
nGraduate of an accredited nursing program.
nActive RN license in Indiana (or NLC license recognized by Indiana).
nMinimum of 2 years nursing experience (home health preferred).
nCurrent CPR certification.
nStrong communication, leadership, and organizational skills.
