Acute Care Case Manager - RN
The case manager position is designed for a dedicated registered nurse with experience in acute care settings. The role focuses on managing patient care effectively throughout their stay, ensuring quality and efficiency in treatment and services provided.
Position Overview
This role entails responsibilities including patient assessment, care planning, and coordination among healthcare teams, to ensure all patient needs are addressed throughout their healthcare journey. The position operates during standard business hours.
Key Responsibilities
Conduct comprehensive patient assessments to develop care plans tailored to individual needs.
Coordinate care across multiple disciplines to ensure efficient service delivery.
Monitor patient progress and adjust care plans as necessary in collaboration with healthcare providers.
Navigate healthcare resources effectively, providing education and resources to patients and families.
Document patient interactions and care plans accurately in compliance with organizational policies.
Qualifications
Active Registered Nurse (RN) license required.
Minimum of one year of experience in an acute care environment.
