Role Description

The Case Manager/Paraprofessional is a part-time, hybrid role based in Decatur, GA, with some work-from-home flexibility. This role involves providing direct support to clients through intake assessments, service coordination, and follow-up to ensure continuity of care. The Case Manager/Paraprofessional will help clients navigate community resources, schedule appointments, and support treatment plans under clinical supervision. Daily tasks include documenting client interactions, maintaining accurate records, communicating with providers and families, and assisting with psychoeducational activities or support groups. The role also involves participating in team meetings, adhering to agency protocols, and promoting ACS’s mission of improving access to mental health services.

Qualifications

Ability to provide compassionate client support, including conducting basic assessments, coordinating services, and assisting with care plans.

Strong communication and interpersonal skills for working with diverse populations, families, and community partners.

Organizational and time-management abilities to handle multiple cases, maintain documentation, and meet reporting deadlines.

Basic computer proficiency, including using electronic records, email, and virtual meeting tools for hybrid work.

Knowledge or interest in mental health, social services, or community resources; prior experience in human services or case management is preferred.

Ability to maintain confidentiality, follow ethical guidelines, and work collaboratively under supervision.

Degree required; coursework or degree in social work, psychology, counseling, or a related field is beneficial.

Willingness to work part time with a flexible schedule in a hybrid setting (on-site in Decatur, GA and remote as assigned). Can move into fulltime role for the right candidate.
