n nJOB SUMMARY

nThe Part Time Case Manager provides supportive case management services to tenants of a permanent affordable supportive housing program under CHI’s master lease consisting of 81 subsidized apartments within a 116-unit multi-dwelling complex. The Case Manager works with tenants to build and enhance independent living skills, including timely rent payments and healthy housekeeping, to promote housing stability.

nPay: $24.04 an hour- approx. $25,376 annually.

nFLSA: non-exempt. Eligible for overtime.

nSchedule: 20 hours- Monday, Friday & Saturday 9am-2pm and Tuesday 2pm-7pm (off Sunday, Wednesday-Thursday).

nLocation: Eastchester neighborhood Bronx, NY

nJOB-RELATED DUTIES – ESSENTIAL FUNCTIONS

nThese duties are essential and specific to the successful implementation of this position.

nTENANT SERVICES

nnnScreen DHS referrals for eligibility and certification for program enrollment.

n

nnConduct tenant intake and needs assessment to determine independent living strengths and weaknesses. Develop independent living plans (ILPs) to ensure housing stability.

n

nnProvide case management to tenants in accordance with ILPs to track progress and milestone achievements.

n

nnCoordinate workshops or provide trainings that promote independent living and housing stability such as budget management, health and nutrition, housekeeping, vocational or employment skills development, job opportunities.

n

nnEngage tenants in supportive and service programs such as Workforce Development and Financial Literacy. Refer tenants to community-based services to meet ILP goals.

n

nnAdvocate on behalf of tenants for entitlements and benefits, employment opportunities, and community-based services.

n

nnLink tenants to employment support services such as resume building, interviewing skills development, and job skills development.

n

nnEnsure proper and timely follow-up for incidents reporting to DHS and CHI.

n

nnSubmit program reports correctly and on schedule to CHI, DHS, and oversight agencies.

n

nnComply with funding contracts, partnership and service agreements, and all applicable city, state, and federal laws, rules and regulations. Complete corrective action plans as directed by Program Director.

n

nnAttend training and obtain certifications per OTDA and DHS rules, including one-time and recurring trainings and certifications.

n

n nTENANCY AND APARTMENT MANAGEMENT

nnnEnsure tenants pay rent timely. Develop rent-based budget schedules.

n

nnAssist tenants with rent payment and rent arrears payment agreements and ensure compliance with rent payment stipulations.

n

nnCounsel tenants to ensure healthy housekeeping and good neighbor behaviors.

n

nnCoordinate with Property Management to ensure essential services including heat, hot water, electric and plumbing are operational; and apartment repairs and maintenance are performed timely.

n nQualifications

nW hat do I need to apply?

nEducation: Bachelor’s Degree in Social Work or Counseling preferred. 3+ years of experience working in supportive housing or a similar field can override the education requirement.

nnnExperience: Two (2) years of experience social work or human services, counseling, or case management.

n

nnCommunication: Professional verbal and written communication skills.

n

nnComputer skills: Professional competency in MS Word, Excel, Outlook and the internet. Experience with CARES and AWARDS prioritized.

n

nnMath Skills: Ability to develop and manage program budgets.

n

nnPhysical Performance: Ability to make unit visits (walk distances, climb stairs, etc.)

n

nnReasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls

n

nnOther Skills and Experience :

n

nnExperience working with homeless and/or formerly homeless individuals and families.

n

nnExperience in needs assessment, crisis intervention, and de-escalation. Behavioral management and motivational interviewing a plus.

n

nnAbility to manage a demanding caseload with competing priorities.

n

nnAbility to work independently and as part of a team. Ability to build and lead a team.

n

nnMust be highly organized and attentive to details, be willing to assume responsibility, work well under pressure, meet deadlines, multi-task, and be flexible.
