Case Manager - Per Diem - Englewood, Colorado

Inverness Rehabilitation Hospital (Englewood, CO) is a 40-bed inpatient rehabilitation facility located in Englewood, Colorado, serving patients throughout the south Denver metropolitan area. The hospital provides comprehensive rehabilitation services for individuals recovering from stroke, brain injury, spinal cord injury, orthopedic conditions, and other complex medical needs requiring intensive therapy. With an interdisciplinary care model and a strong focus on individualized recovery goals, the clinical team works closely with patients and families to promote functional independence and successful transitions home. Located near the Denver Tech Center and the broader South Denver healthcare market, the Inverness facility offers convenient access to specialized rehabilitation care in a growing Colorado community.

We are seeking a Case Manager to join our team of passionate patient caregivers!

Qualifications

Required Skills:

Current clinical licensure strongly preferred.

One year recent relevant experience required. Acute care setting preferred.

Additional Qualifications/Skills:

Demonstrates an understanding of treatment costs and financial support as they relate to quality and efficiency.

Demonstrates critical thinking skills: Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

Utilizes the appropriate leadership skills in delegating, organizing, and educating coworkers and staff.

Knowledge of community resources to meet post-discharge clinical and social needs.

Knowledge of clinical operations and procedures.

Ability to establish and maintain pro-active relationships with internal interdisciplinary team members, local payer, physicians, community service organizations and health care facilities.

Ability to maintain quality, safety, and/or infection control standards.

Demonstrates general computer skills including: data entry, word processing, email, and record management.

Effective organizational and time management skills.

Effective written and verbal communication skills.

Ability to maintain proper levels of confidentiality.

Ability to work closely and professionally with others.

Responsibilities

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital's mission and "Guiding Principles" into daily practice.

Posted Total Compensation The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs. It's not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $ Minimum SalaryUSD $47.00/Hr. Maximum SalaryUSD $47.00/Hr.
