nEquinox is Albany County’s primary provider of services for victims of domestic violence. Our Domestic Violence Programs provide safety for victims of Domestic Violence, including the elderly, and offer the support, assistance and tools they need to escape abusive situations and take control of their lives. Domestic Violence Services are available to all victims and their dependent children, regardless of gender, age, ethnicity, or sexual orientation.
nPosition Summary:The Case Manager provides individualized case management to clients in the Project Break Free rapid re-housing program. Services provided include finding safe and affordable permanent housing, connecting client to needed services, DSS and other service accompaniment, liaison services, information, assessment, education and support. The case manager will assist clients with credit repair, budgeting and other financial education.
nEssential Duties and Responsibilities:
nnnAssesses client needs and abilities, determining level of support needed to provide adequate assistance.
n
nnAccompanies clients to DSS, court, law enforcement, other agencies as needed; advocates for clients both in person and through phone and written contact.
n
nnMaintains accurate case records; completes necessary paperwork and statistic requirements for the agency and funding sources.
n
nnDevelops safety plans with clients.
n
nnAssists clients in finding safe and affordable permanent and/or temporary housing and moving into the same.
n
nnDevelops and maintains positive working relationships with community services and landlords.
n
nnParticipates in staff meetings, as well as individual supervision.
n
nnReports suspected cases of child abuse/maltreatment as a mandated reporter.
n
nnActs in accordance with Equinox and Domestic Violence Services Program mission.
n
nnAbility to model Crisis intervention skills.
n
nnAbility to provide objective supportive listening and guidance.
n
nnAbility to write and speak clearly and concisely.
n
nnAbility to work independently, as well as part of a team.
n
nnAbility to model calm and cooperative behavior.
n
nnAbility to prioritize and complete multiple tasks in an organized manner.
n
nnAbility to maintain strict client confidentiality and engage with a diverse population.

nnPerforms other duties as requested for the enhancement and continuation of programming

n nRequirements
nPhysical Requirements:
nnnAbility to negotiate stairs.
n
nnAbility to lift and carry minimum of 15 lbs.
n
nnAbility to handle basic facility emergencies.
n
nnAbility to drive and transport clients with agency vehicles.
n
n nMinimum Qualifications:
nnnHigh School Diploma or equivalent with a minimum of four years’ experience in human services.
n
nnMust have a clean, valid NYS Driver License, with access to reliable transportation.
n
n nPreferred Qualifications:
nnnAssociates’ Degree in a human service-related field preferred with a minimum of 3 years’ experience in human services OR a Bachelor’s Degree in a human service-related field preferred with a minimum of 2 years’ experience in human services.
n
nnFluency in Spanish and/or an additional language.
n
nnOne year case management/crisis intervention experience
n
nnPrior experience working with domestic violence, sexual assault, elder abuse, human trafficking, mental health or substance abuse populations preferred.
n
nnKnowledge of crime victims’ compensation, state domestic violence regulations, domestic violence dynamics and DSS regulations.
n
n nSchedule:Monday through Friday
