nSUMMARY OF POSITION:
nProvide targeted case management services to child and adult mental health consumers with serious and persistent mental illness. Services offered include functional assessment, service planning, support, network building, linkage of services, monitoring, outreach and problem-solving. The goals of these services is to improve the quality of life, reduce frequency and length of psychiatric hospitalizations and incarcerations, increase independent living ability and strengthen social supports.
nMINIMUM QUALIFICATION(S):
nnnBachelor's Degree
n
nnBasic Life Support (BLS) required prior to completion of orientation
n
nnValid driver's license required.
n
nnPA Act 31 Child Abuse Training Certificate
