nThe Case Manager plays a crucial role in supporting individuals as they navigate available services, helping them access life-changing resources and empowering them on their journey toward wellness and stability.
nIn this role you will:
nnnComplete the timely and accurate documentation in the Uniform Case Record, including but not limited to: referral intake packets and related intake information, referrals and related correspondences, entitlement documentation, logbooks, and other areas as assigned.
n
nnReview all intake information for new referrals and performs initial screening assessments for intake criteria and makes recommendations regarding acceptance or denial.
n
nnPerform Catholic Charities Housing Office intake assessments of each new tenant and then annually thereafter.
n
nnDevelop and implement the ISP and then every 6 months thereafter, reports outcomes and makes needed changes.
n
nnAddress the monthly progress achieved against specific milestones and targets related to housing and referral for services.

n nPosition Details:
nnnCase Manager Salary: $ 21.50
n
nnCase Manager Schedule: Saturday & Sunday (16 hours per week)
n
nnNo subject-to-call shifts required
n
n nQualifications:
nnnEducation:
n
nnAssociate's Degree in Social Work, Bachelor's preferred
n
nnExperience:
n
nn1–3 years’ experience with relevant experience in Human Services or a related field
n
nnBilingual / Spanish preferred
n
nnValid Driver's License along with car insurance, registration, and access to working vehicle required
