Case Manager
Coleman Health Services is seeking a Case Manager for Portage County, Ohio. Come join an energetic, fun, and supportive team that is part of a nationally recognized not-for-profit provider of behavioral health and rehabilitation programs. We are seeking a Case Manager to provide care coordination and other case management services to individuals in Portage County who have mental health and substance use disorders. This position is based in either Ravenna, Ohio or Kent, Ohio.

Duties of Case Manager include: Coordinate ongoing care with internal and external healthcare providers
Assist with symptom monitoring and coping skill development
Assist with making healthcare, service, and community resource referrals
Minimum Qualifications of Case Manager include: BA/BS in mental health/related field or three years' related experience
Have/obtain a NPI number and an Ohio Medicaid number
Have/maintain an excellent driving record
Preferred Qualifications for Case Manager: BA/BS in mental health/related field
Experience in case management
Experience working with adults in a community mental health setting
