Job description
Community Partners in Action seeks a Case Manager for Waterbury Transitional House to conduct assessments, develop service plans, and coordinate residential program services in a supervised, supportive setting.
Bachelor’s degree plus one year of case management experience required; bilingual English/Spanish preferred; valid driver's license and reliable transportation a must; schedule includes two late days per week.
