About This Role
Join CVS Health as a Case Manager Registered Nurse in the field, focusing on serving dual eligible populations. This role is ideal for experienced nurses looking to make a life-changing impact on members enrolled in Medicare and Medicaid.
Highlights
Pay: $66,575 - $142,576/year
Location: Aurora, IL
Residency: Candidate must reside in Kane, LaSalle or surrounding counties
Travel: Willing and able to travel up to 50-75% of the time in Kane, LaSalle and other surrounding counties
What You'll Do
Facilitate the delivery of appropriate benefits and healthcare information
Develop, implement, and support health strategies and policies
Act as a liaison with members, employers, providers, and insurance companies
Implement and coordinate case management activities for catastrophic cases
Assess and analyze medical and vocational status of members
Communicate with members and stakeholders telephonically or in person
Prepare required documentation of case work activities
Consult with internal multidisciplinary teams to maximize health outcomes
Provide educational and prevention information for best medical outcomes
Monitor member progress toward desired outcomes through assessment and evaluation
Requirements
Education
Associate’s Degree in Nursing (REQUIRED)
Bachelor’s Degree in Nursing (PREFERRED)
Experience
3+ years clinical practical experience
Preferred: 2+ years case management, discharge planning, and/or home health care coordination experience
Licenses & Certifications
Active and unrestricted Registered Nurse (RN) in Illinois
