Job description
The Case Manager Registered Nurse is responsible for assessing members' medical and psychosocial needs through regular contact, planning, implementing, and coordinating case management activities to improve health outcomes. This role is ideal for experienced nurses who thrive in a field-based environment and are committed to enhancing member wellness.

Pay: $54,095 - $116,760/year
Location: Owensboro, KY
Schedule: Full-time
Travel: 80-90% in-state travel required
Residency: Must reside in specific counties within the Two Rivers Region (Union, Webster, Henderson, Daviess, Hancock, McLean, Ohio, Butler, Edmonson, Hart, Warren, Logan, Simpson, Allen, Monroe, Metcalfe, Barren)
What You'll Do
Assess members' needs through clinical evaluation and data review.
Develop and implement member care plans addressing biopsychosocial needs.
Conduct comprehensive evaluations of member eligibility and benefit plans.
Utilize motivational interviewing to engage members in their healthcare decisions.
Consult with care partners to overcome barriers and present cases at conferences.
Monitor and revise care plans based on member progress and needs.
Ensure compliance with regulatory and company policies in case management processes.
Travel to meetings and training sessions as required.
Requirements
Education
Associates degree in Nursing or Diploma RN required, BSN preferred
Experience
3+ years of clinical practice experience including patient care coordination
2+ years of experience using personal computers and MS Office Suite applications
Licenses & Certifications
Current unrestricted RN license in Kentucky required
Valid KY driver's license and proof of vehicle insurance required
